We support people during and after big life changes — from hospital discharge and rehab to mental health recovery and leaving care. Our focus is on safety, stability and continuity.
We support the whole family, not just the participant. Our work strengthens routines, rebuilds connection and helps people feel safe, understood and included at home.
We go beyond the NDIS — helping people access housing, health care, employment, education and legal support when needed. We see the full picture and help build a real support network.

We help participants build daily structure, reconnect with their strengths and regain a sense of direction. Whether someone is returning home after hospital, stepping down from mental health services or simply trying to find their feet, we’re here to help.

If your NDIS plan isn’t meeting your needs, we can help. Our team will guide you through the plan reassessment process, making sure the right evidence is in place and your goals are clearly communicated. We’ll work with you to make sure your updated plan reflects the support you actually need.

We offer a free 30-minute consultation for our Support Coordination services to learn more about your situation and talk through what kind of support might help. There's no pressure or obligation — it's simply a chance to see if we're the right fit. For all other services, feel free to give us a call or fill out our referral form and we'll be in touch to schedule a free initial chat.
Yes. We specialise in post-discharge support, especially after hospital, rehab or mental health care. We help put the right supports in place at home and make sure nothing falls through the gaps.
Absolutely. We have experience supporting children, teens and their families — especially in complex situations like trauma, behavioural needs or transitions from care. We work closely with parents, carers and extended family.
We’re based in Mulgrave and work across the Casey, Monash and Wyndham council areas — covering both the southeastern and western suburbs of Melbourne. We’re also expanding into the eastern and inner northern suburbs. Our team is mobile and flexible, and we’re open to supporting people in new areas. If you’re not sure if we cover your area, just ask — we’ll let you know what’s possible.
No. While we work within the NDIS, we also help connect people to housing, employment, health, legal and other community supports. If it helps build stability or improve life, we’ll try to find a way to make it happen.
