ABOUT US
support grounded in real experience
Yolanda is a qualified social worker with broad experience across government and community sectors, including disability, Out of Home Care, and Child, Youth and Family Services. She understands the system inside and out — not just from a policy perspective, but through the real challenges participants and families face every day. Her approach is built on empathy, practical advocacy, and a whole-of-life view of support.
Her background includes hands-on experience with complex family situations, trauma, and care environments, giving her a solid understanding of how to support people facing tough circumstances. Stability, safety, and clear, reliable support are always her priority.
Through Linkr Care, Yolanda leads a team helping people navigate the NDIS and break through the barriers that often hold things up. We don't just stop at funded supports — we help connect people with health services, housing, and other community supports, making sure they’ve got the right team around them.
The essence of happiness is having hope, purpose, and connection. That belief is what led me to create Linkr Care — to offer more than just services, but real support that helps people rebuild confidence, feel empowered, and find meaning in their everyday lives. We walk alongside each person and their family with empathy, cultural understanding, and a deep commitment to helping them feel seen, supported, and truly valued.
OUR TEAM
We speak Cantonese, Mandarin, Teochew, Hindi, Marathi and Gujarati.
Everyone at Linkr Care brings hands-on experience in the work that matters — from complex personal care and mental health support to day-to-day routines, mobility and community access. Our team includes professionals from nursing, aged care, disability and family services. We also reflect the communities we work with — speaking multiple languages and understanding cultural expectations from lived experience, not just training. Whether someone needs help with daily tasks or is facing something more complex, we focus on what’s practical, respectful and actually helpful.

OUR APPROACH
Support should feel steady, not overwhelming. We take time to understand the whole picture — not just what’s written in a plan — and build trust with every person and family we support.

WHO WE ARE
Linkr Care supports children, teens and adults with day-to-day NDIS services as well as during bigger life transitions. From help around the home to navigating systems, rebuilding routines or working through complex family needs — we bring calm, capable support that fits real life. Everything we do is culturally responsive, trauma-informed and focused on the whole family, not just the participant.
WHAT GUIDES US
To support people with disability and their families through steady, inclusive services that strengthen daily life, relationships and long-term wellbeing.
A community where every person — no matter their background or support needs — feels respected, understood and meaningfully included in the life they choose.
